Due to continued growth, I have a very exciting opportunity for a registered Health and Social Care professional to join a market leading Case Management provider, as a Case Manager.
The organisation offers specialist bespoke rehabilitation and lifestyle programmes to both Adults and Children and Young People, specialising in the below:
- Brain Injury
- Spinal Injury
- Children & Young People
- Neurological conditions
- Complex orthopaedic multi-trauma
As a Case Manager, you'll assess organise and manage bespoke care and rehabilitation packages for Clients who have acquired a brain injury, spinal injury or life-changing physical disabilities usually due to an accident or a birth injury.
Full, extensive training will be provided from day one.
Salary - Starting salary is £30,000.00
On successful completion of induction stake holder pension currently paid at 3% plus Medicash
Hours - 35-hour week, Monday - Friday 9-5.
Location- Bromborough. This organisation supports clients throughout the North West.
Ideally, cases will be matched up to as close to home as possible.
We currently have positions across both our specialist Adult and Children and Young People teams.
You will be responsible for completing a thorough and detailed assessment of a Client's needs, taking a holistic approach.
You will be working closely with your Client to maximise their potential, through the provision of services bespoke to their needs, in addition to supporting their families through the process.
You will identify their rehabilitation, care, emotional and social needs. You'll implement a rehabilitation and appropriate care package, if and when required, and continually review and assess it, to ensure that it continues to meet the client's goals.
The care package will be delivered by an external provider or through supporting the client to directly recruit appropriate staff and to ensure that the care plan is followed.
You'll work within a Multi-disciplinary team consisting of Nurses, OTs, Physios, Social Workers and other Health and Social Care professionals and be able to share knowledge and experiences with others within the organisation.
If you are an experienced and active Health and Social Care professional who has an interest in Case Management, you can apply, providing you have the below:
- You are a qualified healthcare professional, registered to practice in the UK - Occupational Therapist, Nurse, Social Worker, Physiotherapist, Psychologist
- You will need a minimum of 2 years post qualification experience.
- You will need to be a driver with your own vehicle - mileage is paid at 45p a mile.
- For the Children and Young People role, relevant clinical experience in a paediatric setting is essential
- Ideally you will have worked in the community before and/or have previous experience of working with clients following Brain Injury.
- You are passionate about your work, organised and have an eye for detail
- You possess the ability to clearly explain and present assessment findings and recommendations
- You are highly motivated and self-directed, while possessing excellent team working skills
