We are actively seeking experienced Band 6 RMN's to join the Community Nursing Team in the NHS in West Essex to cover Epping, Harlow & Uttlesford. We are looking for candidates who are able to work fulltime & part time over the 7 days per week which this service covers.

About the Role:
The service has been developed in partnership to provide a full multidisciplinary crisis response to individuals in their own home or care setting, to support the NHS longterm ambition of preventing all unnecessary hospital admissions and ensuring delivery of care by the right person, in the right place at the right time.
You will work as a member of the Urgent Care Response Multi Disciplinary Team in providing high quality, evidence-based, holistic care to patients. You will assess, plan, implement and evaluate packages of treatment and care based upon individual service user needs, under the guidance of the Advanced Nurse Practitioner as per NICE guidance.
The Urgent Care Response Team is a new service to provide a 2 hr response to patients who are in a mental or physical health crisis, who without the 2 hr response are at risk of being admitted to an inpatient ward inappropriately. The MDT work collaboratively to ensure an out of hospital treatment pathway is considered as an alternative to admission if clinically appropriate.
As required by your manager, you will have responsibility for the day to day supervision and management of lower banded staff within your professional group and team locality.
Duties will include:
Reviewing and planning care for patients
The implementation of care and treatment plans
Liaising with other professionals to deliver care to patients, including liaising with families.

You must be a Band 6 RMN with at least 6 months relevant and recent experience
You must hold a valid NMC PIN
