This exciting post will involve work with patients who have complex mental health needs and who offend.

The post holder will bring knowledge of social policy and expertise in Social Work practice to bear upon the needs of the clients and customers (internal and external). They will have responsibility for ensuring and maintaining a quality Social Work service to patients and their families.


Our client a psychiatric services, operate throughout England and Wales caring for people with mental illness, learning disabilities and acquired brain injuries.

The business has four types of customer: patients, staff, purchasers and external agencies.


Patients may suffer from mental illness, personality disorders and/or learning disabilities. Patients at the Unit are detained under the Mental Health Act. Referrals are taken from many sources including psychiatric hospitals, prisons and the courts.


The workforce of the Organisation is multi-disciplinary; a significant proportion of those employed are in a clinical discipline with other departments providing supporting services.


Professional:

  • To meet the standards set out in the General Social Care Councils Code of Practice for Social Care Workers.
  • Be registered with the Health and Care professions Council.

Clinical:

  • Undertake a comprehensive assessment of the patients social care needs.
  • Have full responsibility for managing a caseload.
  • To actively participate in the MDT care planning process.
  • Contribute towards completion of patients CPA documents providing comprehensive assessment reports when required.
  • To ensure an accurate holistic and up to date social history is available for each patient, which offers formulations and recommendations.
  • When it is part of an agreed care plan focussed casework with individual patients using a range of interventions.
  • To facilitate group work jointly with other disciplines.
  • With the patient's permission initiate visit and maintain contact with patients families, carers and/or significant others offering support, information and where relevant to the care plan joint family and patient sessions which are enabling.
  • To liaise with Social Services Departments to ensure that reports and attendance by a representative are available for Mental Health Reviews Tribunals. Where necessary, prepare reports and attend hearings in collaboration with Social Services Departments.
  • To play a co-ordinating role in the process of section 117 Mental Health Act 83 aftercare plans with the relevant community agencies which meet the needs of the patient and takes full account of risk factors.
  • In conjunction with the responsible Social Services Department or Integrated NHS assist in the assessment of suitable after care placements working across professional boundaries.
  • To maintain accurate and up to date records of their social work inputs to the care plan including Section 117 care planning and transfer summaries, which reflect the patient's views.
  • Ensure that patients have access to independent advice on financial legal and advocacy matters.
  • To attend relevant Multi-Disciplinary Team meetings as required.

Management &Departmental Responsibilities:


    • To undertake patient and staff related investigations
    • Identify the training needs of all social work staff with a view to enhancing opportunities for individual professional development within the department and contribute to the in service training programme as required.
    • To actively participate in the development of the team, taking responsibility for specialist clinical areas of patient need by delivering therapeutic expertise.
  • To attend and actively contribute towards Social Work Team meetings.
  • Review and evaluate regularly with the social work team the service offered to ensure the service provided meets the needs of the patients and is in line with Social Work Practice guidelines.

The job holder will be required to attend hand-over meetings, ward rounds, CPAs, case conferences and ward meetings, including community meetings and reflective practice meetings. They are expected to be aware of, and comply with, all policies and guidelines relating to staff at the North London Clinic and in particular those dealing with confidentiality, safety and security.


To maintain written and electronic records of clinical activity, in accordance with the Health and Care Professions Council standards and company policy.
