Are you an experienced senior care assistant passionate about making a difference? Join a leading home care provider in Cardiff as a Care Coordinator, and help deliver exceptional care across the Vale of Glamorgan and Cardiff. This impactful role offers a rewarding opportunity to coordinate and support a dedicated team of Care Assistants, ensuring seamless care delivery for our clients.
Key responsibilities include:
- Scheduling and matching Healthcare Assistants to client care needs
- Ensuring comprehensive coverage for all shifts and care packages
- Managing new referrals and updating care plans as required
- Maintaining accurate records and reports in collaboration with the office team
- Supporting Care Assistants by providing necessary resources and guidance
- Participating in the on-call rota, including one weekday and one weekend in four
What we’re looking for:
- A senior care assistant with at least 1 year of experience in the care sector
- Previous experience as a Care Coordinator is preferred
- QCF qualification in Health & Social Care
- Social Care Wales registration (desirable but not essential)
- Full UK driving licence and access to own vehicle
- Ability to stay organised and perform under pressure
- Compassionate nature with a genuine desire to improve client wellbeing
