We are seeking a highly motivated Care Coordinator to join a leading specialist provider of home care services in Cardiff and the Vale. As a Care Coordinator, you will be the heartbeat of the service, ensuring smooth scheduling and outstanding support for our Care Assistants and customers across the Vale. This is your chance to make a genuine impact every day, while enjoying excellent rewards and being part of a supportive, people-first team.

Key Responsibilities:

  • Scheduling and matching Care Assistants to customers in Cardiff and the surrounding area
  • Ensuring seamless rota cover and smooth care delivery across the region
  • Handling new referrals and updating care plans accordingly
  • Working closely with the office team to maintain accurate records and reports
  • Supporting Care Assistants to ensure they have what they need to deliver high-quality care
  • Participating in the on-call rota (1 weekday per week and 1 weekend in 4)

Requirements:

  • Minimum 1 year's experience in the care sector (Senior Care Assistant level)
  • Previous Care Coordinator experience
  • Level 3 Health & Social Care qualification (required)
  • QCF in Health & Social Care & Social Care Wales registration (desirable but not essential)
  • Full UK driving licence and access to your own vehicle
  • Ability to stay organised, calm, and focused under pressure
  • Strong communication and people-management skills
