Are you passionate about making a positive difference in people's lives? A leading specialist provider is seeking a dedicated Mental Health and Wellbeing Coach to join their team in Gwent. As a Mental Health and Wellbeing Coach in Gwent, you will work closely with a multidisciplinary team to deliver tailored support that empowers service users to achieve their goals. Key Responsibilities:
- Provide practical support, including assistance with Care and Treatment Plan reviews, medical appointments, community activities, and daily living skills such as money management and tenancy support.
- Support service users in emotional regulation, problem solving, and relationship building.
- Develop and implement personalised support plans based on Structured Clinical Management (SCM) principles.
- Promote service user choice and control, ensuring they are active partners in their support.
- Engage in reflective practice, team supervisions, and meetings to foster continuous improvement.
- Maintain accurate records, incident reports, and escalate safeguarding concerns as necessary.
- Collect feedback and contribute to service reviews and reporting processes.
Requirements:
- Proven experience in front-line support of individuals with mental health needs, ideally across a broad geographical area such as Gwent.
- Full driving licence and ability to travel across Gwent and inpatient settings.
- Ability to respond quickly and positively to individual needs, demonstrating flexibility and proactive support.
- Strong relationship-building skills, with the ability to establish trusting therapeutic relationships.
- High emotional intelligence, resilience, and empathy.
- Experience working with individuals with complex mental health difficulties and challenging behaviour.
- Ability to work independently and adapt to changing priorities.
Desirable:
- Knowledge of PIE, ACEs, and strength-based approaches.
- Experience advocating for service users’ needs.
- Willingness to reflect on feedback and pursue personal development.
- Understanding of community and third sector organisations in Gwent.
- Welsh language skills are advantageous.
