Are you passionate about making a positive difference in people's lives? A leading specialist provider is seeking a dedicated Mental Health and Wellbeing Coach to join their team in Gwent. This role offers a rewarding opportunity to support individuals transitioning from secure or residential settings into the community, promoting independence and wellbeing. As a Mental Health and Wellbeing Coach in Gwent, you will work closely with a multidisciplinary team to deliver tailored support that empowers service users to achieve their goals. This is an excellent chance to contribute to a forward-thinking service that prioritises person-centred care and recovery.

Key Responsibilities:

  • Provide practical support, including assistance with Care and Treatment Plan reviews, medical appointments, community activities, and daily living skills such as money management and tenancy support.
  • Support service users in emotional regulation, problem solving, and relationship building.
  • Develop and implement personalised support plans based on Structured Clinical Management (SCM) principles.
  • Promote service user choice and control, ensuring they are active partners in their support.
  • Engage in reflective practice, team supervisions, and meetings to foster continuous improvement.
  • Maintain accurate records, incident reports, and escalate safeguarding concerns as necessary.
  • Collect feedback and contribute to service reviews and reporting processes.

Requirements:

  • Proven experience in front-line support of individuals with mental health needs, ideally across a broad geographical area such as Gwent.
  • Full driving licence and ability to travel across Gwent and inpatient settings.
  • Ability to respond quickly and positively to individual needs, demonstrating flexibility and proactive support.
  • Strong relationship-building skills, with the ability to establish trusting therapeutic relationships.
  • High emotional intelligence, resilience, and empathy.
  • Experience working with individuals with complex mental health difficulties and challenging behaviour.
  • Ability to work independently and adapt to changing priorities.

Desirable:

  • Knowledge of PIE, ACEs, and strength-based approaches.
  • Experience advocating for service users’ needs.
  • Willingness to reflect on feedback and pursue personal development.
  • Understanding of community and third sector organisations in Gwent.
  • Welsh language skills are advantageous.

This is a full-time, fixed-term contract until August 2027, working on a six-week rolling rota that includes day, evening, weekend, and night shifts. If you are committed to supporting mental health and wellbeing in the Gwent community, this role offers a meaningful career opportunity.
