Job Description:
Our client, a leading healthcare provider, is seeking a dedicated Mental Health and Wellbeing Coach to join their team in Gwent. This role offers an exciting opportunity to make a positive impact by supporting individuals transitioning from secure or residential settings into the community, promoting independence and recovery.
Key Responsibilities:
As a Mental Health and Wellbeing Coach based in Gwent, your daily duties will include:
- Supporting daily living and independence through activities such as Care and Treatment Plan reviews, medical appointments, and community engagement.
- Assisting with practical skills like money management and tenancy support.
- Providing emotional regulation, coping strategies, and problem-solving support to service users.
- Developing personalised support plans grounded in Structured Clinical Management (SCM) principles, ensuring service users have choice and control.
- Facilitating transitions from secure or residential settings into the community, focusing on recovery and independence.
- Collaborating with a multidisciplinary team, participating in reflective practice, team meetings, and maintaining accurate records.
- Escalating safeguarding concerns and collecting service user feedback for continuous improvement.
- Travelling across Gwent and inpatient settings, working independently within a rotating shift pattern that includes day, evening, weekend, and night shifts.
Requirements & Qualifications:
To be successful as a Mental Health and Wellbeing Coach, you will need:
- A full UK driving licence and the ability to travel across Gwent and inpatient settings.
- Experience supporting individuals with complex mental health needs in community settings.
- Strong relationship-building skills and the ability to work independently with flexibility.
- Knowledge of positive behaviour support (PBS), person-centred care planning, and emotional literacy.
- Excellent communication skills, emotional intelligence, and resilience.
- Training in Structured Clinical Management (SCM) or similar frameworks is desirable but not essential; training will be provided.
