Job Description

Mental Health and Wellbeing Coach roles are available within our client’s Bespoke Repatriation Service in Gwent, South Wales, supporting people to transition from secure or residential settings into the community with their own tenancy. You’ll be part of a responsive, person-centred service delivered in partnership with local health services, with training and ongoing supervision provided.

Key Responsibilities

As a Mental Health and Wellbeing Coach based in Gwent, your daily duties will include:

  • Providing practical, recovery-focused support including Care and Treatment Plan reviews, medical appointments, community activities, emotional regulation, problem-solving, relationship building, and daily living skills (e.g., money management and tenancy management).
  • Developing and delivering tailored support plans underpinned by Structured Clinical Management (SCM), working closely with the wider intensive case management team.
  • Maintaining accurate daily records and incident reports, escalating safeguarding concerns appropriately, and contributing to service measurement and improvement through feedback and reporting.
  • Working flexibly across Gwent on a rolling rota to support a 24/7 service, including days, evenings, weekends, and nights.

Requirements & Qualifications

To be successful as a Mental Health and Wellbeing Coach, you will need:

  • Front-line support experience with service users, ideally across a wide geographical area.
  • Proven experience supporting individuals with complex mental health difficulties and/or challenging behaviour, including crisis planning and goal-focused support.
  • Full UK driving licence and ability to travel across Gwent and to inpatient settings further afield.
  • Strong emotional intelligence: resilient, empathic, non-judgemental, and confident in building therapeutic relationships and setting appropriate boundaries.

Desirable: understanding of Psychologically Informed Environments (PIE)ACEs, strength/restorative approaches, advocacy experience, knowledge of community/third-sector services across Gwent, and Welsh language skills.
