Job Description:

Our client, a leading healthcare provider, is seeking a dedicated Mental Health Support Worker to join their team in Abergavenny, Wales. This role offers an exciting opportunity to support individuals transitioning from secure or residential settings into community living, providing tailored, person-centred care.

Key Responsibilities:
As a Mental Health Support Worker based in Abergavenny, your daily duties will include:

• Providing practical support such as assisting with Care and Treatment Plan reviews, medical appointments, community activities, and developing daily living skills like money management and tenancy support.
• Supporting service users to have choice and control in their support plans, ensuring they are active partners in their care.
• Engaging in reflective practice, team supervisions, and promoting standards underpinned by a Psychologically Informed Environment.
• Updating records, incident reports, and escalating safeguarding concerns as necessary.
• Gathering feedback through measurements and resident input to inform service reviews and improvements.

Requirements & Qualifications:
To be successful as a Mental Health Support Worker, you will need:

• Experience in front-line support of individuals with mental health needs, ideally across a broad geographical area such as Gwent.
• A full UK driving licence and the ability to travel across Gwent and inpatient settings.
• Strong emotional intelligence, resilience, and the ability to build trusting therapeutic relationships.
• Experience working with individuals with complex mental health difficulties or challenging behaviour.
• Flexibility to work on a six-week rolling rota, including days, evenings, weekends, and nights.

Desirable:

• Knowledge of PIE, ACEs, and restorative approaches.
• Experience advocating for service users’ needs.
• Welsh language skills.
