Are you passionate about making a difference in the lives of vulnerable individuals? We are seeking a dedicated Outreach Support Worker to join a leading specialist provider in Monmouthshire. This role offers an exciting opportunity to support people experiencing homelessness, including those in emergency accommodation and rough sleepers, helping them secure and maintain suitable housing. The Outreach Support Worker in Monmouthshire will play a vital role in delivering compassionate, person-centred support across the community.
Key Responsibilities:
- Support individuals affected by homelessness across Monmouthshire, including those in temporary accommodation and rough sleeping.
- Conduct needs assessments and develop tailored support plans in collaboration with service users.
- Provide intensive outreach support to help clients secure long-term housing solutions.
- Undertake risk assessments and safety planning to ensure client and staff safety.
- Work within multi-agency hubs, liaising with social services, police, housing providers, and other partners.
- Maintain accurate case notes and support documentation using electronic case management systems.
- Assist clients in building community support networks to promote successful independent living.
- Participate in on-call duties and team meetings to ensure continuous support coverage.
- Contribute to the monitoring and evaluation of support activities to improve service delivery.
Requirements:
- At least 1 year of experience working with individuals experiencing homelessness or in a related support setting.
- Strong ability to build positive relationships with vulnerable individuals.
- Experience in assessing needs, risks, and developing support plans collaboratively.
- Excellent communication skills, both verbal and written.
- Ability to work effectively as part of a team and independently under pressure.
- Knowledge of social legislation, welfare rights, and health and safety regulations.
- Full driving license and access to a vehicle.
Desirable:
- Educated to A-Level / GCSE standard or equivalent.
- Experience working within a psychologically informed environment.
- Fluency in Welsh.
- Experience liaising with statutory and voluntary agencies.
