About the Role: 

As a Housing Outreach Support Worker, you will provide essential housing-related support across Monmouthshire. Your days will be dynamic and impactful, combining intensive support within temporary accommodation (B&B, hotels, hostels, night shelters) with crucial assertive outreach to individuals who are rough sleeping. While based primarily in Monmouthshire, some travel to our Newport office will be required.

Key Responsibilities:

  • Support individuals affected by homelessness to find, secure, and maintain appropriate accommodation, including within the private rented sector.
  • Conduct comprehensive assessments of housing needs, risks, and develop robust risk management and safety plans in line with organisational policies.
  • Provide intensive support to residents in temporary accommodation, facilitating their transition to longer-term housing solutions.
  • Work effectively within multi-agency Hubs, liaising with Social Services, Police, Housing Options Officers, DWP, and other statutory and voluntary agencies to provide holistic support.
  • Assist individuals in building and sustaining informal community support networks to promote successful independent living.
  • Ensure accurate and timely documentation of all support notes and service user information using electronic database systems 
  • Participate in the organisation's vital tiered on-call rota.
  • Engage actively in team meetings, supervision, and continuous professional development opportunities.

Essential Qualifications & Experience:

  • At least 1 year of experience working directly with individuals experiencing homelessness or in a related support setting.
  • Knowledge of the principles and ability to conduct comprehensive assessments of need, risk, and develop person-centred support plans.
  • Proven experience in successful partnership working with various agencies.
  • Ability to build and sustain positive, non-judgemental relationships with vulnerable individuals.
  • Strong ability to assess and respond to static and dynamic risks, ensuring robust risk management plans.
  • Excellent written and verbal communication skills.
  • Proficiency in computer skills
  • Ability to work on own initiative, prioritise workload, and manage pressure effectively.
  • A professional approach, demonstrating strong professional boundaries.
  • Understanding of and commitment to: Equal Opportunities, psychologically informed environments, confidentiality, and Health & Safety regulations.
  • Full UK Driving Licence and access to a vehicle are essential.

Desirable Qualifications & Experience:

  • Experience working within a Psychologically Informed Environment (PIE) framework.
  • Experience of successfully liaising with a wide variety of agencies (e.g., Social Services, Police, Housing, DWP, Health Professionals).
  • Experience with case management systems and IT.
  • Ability to speak Welsh.
